Every year, when I attend congresses like IMCAS Paris and AMWC, I can tell which topic is “in fashion” by how full the halls are. For a few years now, one subject has overshadowed all the others: exosomes. But this isn’t just a congress fad; I see the same shift in the clinic. Patients no longer say “fix my face,” they say “renew my skin from within, naturally and long-lasting.” Exosome therapies are precisely the answer to that sentence.
In this article I want to explain — soberly, through a physician’s eyes — what exosomes are, why they’re talked about so much, how they give the best results, and where their scientific basis really stands.
What is an exosome, and how does it work?
Exosomes are tiny “messenger packets,” 30–150 nanometres in size, that cells send to one another. They carry growth factors, proteins and regulatory molecules called miRNAs. So they don’t really carry a “substance,” they carry a set of instructions — they tell cells what to do.
In the skin, this means: the aim is for exosomes to stimulate fibroblasts (the cells that produce collagen), support collagen and elastin production, balance inflammation and assist tissue repair. Their fundamental difference from classic filler or botox — which “replace what’s missing from the outside” — is this: with exosomes the aim is to remind the skin of its own production. What is targeted isn’t an externally added appearance but skin quality coming from within.
Why is it talked about so much in 2026?
Two currents met at the same point.
